Welcome to the Premier Health Employee Plan Biometric Screenings Registration!

As a reminder, ALL Premier Health employees are eligible for a biometric screening.

On-site screenings will be held throughout Premier Health campuses – January through October:

Registration below is simple - provide your Date of Birth and Employee ID number to get started. Follow the prompts to complete your registration.

You will be receiving a fingerstick screening for total cholesterol, HDL, LDL, cholesterol ratio, and hemoglobin A1c. Your blood pressure, height, weight, and Body Mass Index will also be measured. No fasting is required for this screening.

Registration will close 48 hours before the screening date.
